Includes:
• Light vital sign check (BP, pulse, temperature if needed)
• Medication reminders or pill organizer setup
• Basic hygiene prompts and/or meal or hydration prep
• Emotional support, safety check, and routine encouragement
• Care Partner will also monitor for:
• Changes in cognition or alertness
• Signs of swelling, weakness, or shortness of breath
• Any unusual changes in mood, behavior, or strength — all of which could signal a need
for early medical attention
Regular visits help detect small changes before they become emergencies.

support, or cognitive monitoring — but do not require live-in care.

✔ Wellness checks, vitals, med setup, hygiene support
✔ Light meal prep, redirection, companionship
✔ Care Partner observations of changes in:
• Cognition
• Swelling, weakness, or fall risk
• Behavior or signs that may indicate medical concern
Helps prevent unnecessary hospitalizations by identifying changes early.
